Veteran Liaison – Home Care Referral & Enrollment Specialist

ComForCare Home Care – San Diego South Bay and Coronado is looking for a Veteran Liaison to help local veterans and their families navigate VA-funded in-home care benefits. This is a relationship-driven, lead-management role. You'll be the first point of contact for veteran leads generated through our marketing efforts, and you'll guide each family from first contact through enrollment and the start of care. What you'll do: - Follow up promptly with new veteran leads by phone, text, and email - Explain VA home care benefits clearly — Home Health Aide and Aid & Attendance programs — and help families understand which one fits their situation - Walk veterans not yet enrolled in VA healthcare through the enrollment process - Guide enrolled veterans through the steps to get Home Health Aide services approved (VA doctor's appointment, case worker review, authorization) - Build and maintain relationships with VA Social Workers and case managers at local VA facilities - Track leads through our follow-up system and keep clear records of where each family stands - Coordinate warm handoffs to our care team once a client is ready to start service What we're looking for: - Experience with VA benefits, military/veteran services, or healthcare referral/enrollment work - Existing relationships with VA Social Workers, case managers, or veteran service organizations strongly preferred - Comfortable making outbound calls and managing a multi-step follow-up process - Warm, patient, and genuinely comfortable talking with veterans and their families about sensitive care needs - Organized — able to manage multiple leads at different stages at once - Prior sales, patient advocacy, or care coordination experience a plus Compensation: Commission-based, discussed during interview. This role begins as a paid trial with the opportunity to extend based on results. This is a flexible, largely independent role — you'll set your own schedule around calls and appointments, with regular check-ins with our office.
