    Job Summary: The Behavioral Health Case Manager at is responsible for providing comprehensive case management services to patients with behavioral health needs. This position works closely with healthcare providers and community resources to ensure patients receive timely and coordinated care, addressing mental health, substance use disorders and patients Social Determinants of Health.

    Essential Duties:

    • Conduct initial assessments of patients' behavioral health needs, including mental health disorders, substance use, and social determinants of health, to develop individualized care plans.
    • Collaborate with interdisciplinary healthcare teams to coordinate and monitor the delivery of behavioral health services.
    • Provide ongoing support and advocacy to patients and their families to navigate the healthcare system and provide warm handoffs to higher levels of care, as well as behavioral health resources that are not provided by Opsam.
    • Facilitate communication between primary care providers, psychiatrists, therapists, and other healthcare professionals to ensure continuity of care for patients with behavioral health needs.
    • Conduct regular follow-up with patients to monitor progress in treatment, address barriers to care, and support adherence to treatment plans.
    • Assist patients in accessing social services, housing assistance, financial resources, and other support services to address underlying social determinants of health.
    • Maintain accurate and up-to-date documentation of patient interactions, care plans, and referrals in the electronic health record (EHR) system.
    • Participate in case conferences, treatment planning meetings, and other multidisciplinary forums to discuss patient progress and coordinate care efforts as needed.
    • Provide crisis intervention services and support to patients experiencing acute mental health or substance use crises, including coordinating emergency services and referrals.
    • Stay informed about changes in healthcare policies, regulations, and best practices related to behavioral health case management and care coordination.
    • Collaborate with community organizations, government agencies, and advocacy groups to develop partnerships and enhance access to behavioral health resources for patients.
    • Uphold ethical standards and confidentiality guidelines in all aspects of job performance and patient care.

     

     

    Qualifications:

    • Bachelor's degree in social work, psychology, counseling, nursing, or related field
    • Previous experience in case management, care coordination, or behavioral health services preferred
    • Strong communication, interpersonal, and organizational skills
    • Strong knowledge of community resources throughout San Diego County
    • Ability to work effectively in a fast-paced, team-oriented environment
    • Proficiency in electronic health record (EHR) systems and Microsoft Office applications
